Sexually transmitted infections, including HIV, in the Netherlands in 2012

English Abstract
In 2012, more people were tested for sexually transmitted infections (STIs) at the STI clinics than in previous years, and more people were diagnosed with an STI. The percentage of people with a chlamydia or gonorrhea infection showed an especially marked increase. This trend is visible at the STI clinics as well as in general practices. Effective STI monitoring remains essential for the identification of relevant trends and of emerging STIs in high-risk groups.

STI clinics offer high-risk groups a low-threshold opportunity to test for STI and offer treatment when an STI is diagnosed. The attendees at the STI clinics in 2012 consisted mainly of people below the age of 25 years, people from areas in which STI/HIV is common and men who have sex with men (MSM). The number of consultations increased by 7 per cent compared with 2011.

Chlamydia
Chlamydia remained the most diagnosed STI in 2012, with a slight increase in the percentage of people with this infection compared with 2011 (12.2 per cent versus 11.5 per cent). Chlamydia was primarily diagnosed in heterosexuals, both men and women, below the age of 25 years. In 2012, a remarkable increase in the number of lymphogranuloma venereum (LGV) infections, an aggressive variant of chlamydia, was observed. Currently, LGV is mostly diagnosed in MSM with an HIV infection. LGV causes an infection of the lymphenodes, which prolongs the treatment.

Gonorrhoea
In 2012, the percentage of people with a gonorrhoea infection also increased in comparison with 2011 (3.6 per cent versus 3.2 per cent). Gonorrhoea was mostly diagnosed in MSM. It remains important to monitor the resistance of the gonorrhea bacteria against the antibiotic currently recommended in the Netherlands, a third-generation cephalosporine. In several European countries rising rates of resistance to this antibiotic are being observed.

HIV
In 2012, the number of people with an HIV infection diagnosed in the STI clinics decreased slightly compared with 2011 (358 versus 415). At the HIV treatment centres, the number of HIV-infected patients entering care has been more or less stable in recent years. However, more newly diagnosed people of 50 years of age or older have been registered in the past few years.

In 2012 hebben meer mensen zich bij een soa-poli laten testen op een seksueel overdraagbare aandoening (soa) dan in voorgaande jaren. Ook is bij meer mensen een soa geconstateerd. Vooral het percentage mensen met een chlamydia- en gonorroe-infectie nam toe. Deze tendensen zijn zichtbaar bij zowel de soa-poli's als de huisartsen. Een goed functionerende soa-monitoring blijft daarom essentieel om zicht te houden op relevante trends en opkomende soa binnen groepen die een grotere kans hebben deze infectie op te lopen.

De soa-poli's bieden hoogrisicogroepen een laagdrempelige mogelijkheid om zich te laten testen en verstrekken medicatie als een soa wordt vastgesteld. De bezoekers van de soa-poli's waren in 2012 voornamelijk jongeren tot 25 jaar, migranten uit landen waar soa's en hiv veel voorkomen en mannen die seks hebben met mannen (MSM). Het aantal consulten is met 7 procent toegenomen ten opzichte van 2011.

Chlamydia
Bij de soa-poli's was chlamydia opnieuw de meest gediagnosticeerde soa in 2012, met een lichte stijging ten opzichte van 2011 (12,2 versus 11,5 procent). Chlamydia werd het meest vastgesteld bij heteroseksuelen, zowel mannen als vrouwen, onder de 25 jaar. Opmerkelijk was de sterke stijging van lymphogranuloma venereum (LGV), een agressieve variant van chlamydia. LGV is tot nu toe voornamelijk vastgesteld bij MSM die een hiv-infectie hebben. LGV veroorzaakt een ontsteking van de lymfeklieren, waardoor de behandeling langer duurt.

Gonorroe
Ook het percentage mensen met een gonorroe-infectie bij de soa-poli's is in 2012 toegenomen ten opzichte van 2011 (3,6 versus 3,2 procent). Deze soa werd het meest gediagnosticeerd bij MSM. Het blijft belangrijk om te volgen of de gonorroebacterie resistent raakt tegen het antibioticum dat nu in Nederland als eerste keus aanbevolen wordt, een derde generatie cefalosporine. In diverse Europese landen is deze resistentie waargenomen.

Hiv
Het aantal personen met een hiv-infectie, gediagnosticeerd bij een van de soa-poli's, is in 2012 licht afgenomen (358 versus 415 in 2011). Het aantal mensen dat jaarlijks bij een hiv-behandelcentrum onder behandeling komt, blijft min of meer stabiel. Er werden wel meer mensen van 50 jaar of ouder nieuw gediagnosticeerd met hiv in de afgelopen jaren.
