English Abstract
Marriage between close biological kin (consanguineous marriage) is one of the factors that increase the risk of congenital disorders. Other factors are, for instance, advanced maternal age, diabetes in pregnant women or previous genetic diseases in the family. Children of consanguineous couples are especially at risk for very rare autosomal recessive disorders. The contribution of these very rare disorders to the total amount of birth defects and/or genetic diseases is very small, however. A limited number of children with an autosomal recessive disorder will die from the disorder, with the highest risk on mortality in their first year of life. In absolute numbers the contribution of autosomal recessive disorders to perinatal and infant mortality in the Netherlands is very low. No reliable information is available on the burden of disease by the autosomal recessive disorders that do not lead to mortality. There are no representative data available on the prevalence of consanguinity in the Netherlands. By using incidentally available and sometimes indirect information it may be concluded that consanguinity is more common in the Turkish and Moroccan population and in some small religious groups in the Netherlands. Although the absolute numbers are small, these specific disorders do have a large impact on the affected families. There is increased awareness of the need for timely counseling and screening for genetic risks, including consanguinity and the risk within specific parts of the population on less rare autosomal recessive disorders (such as hereditary anaemia). Ideally, all parents to be should, preferably before a pregnancy, have access to counseling on increased risks of having a child with a genetic disorder and on possible preventive measures.

Bloedverwantschap tussen ouders (consanguiniteit) is een van de risicofactoren die de kans op aangeboren aandoeningen verhogen. Andere risicofactoren zijn bijvoorbeeld moederschap op oudere leeftijd, diabetes bij de moeder of erfelijke aandoeningen in de familie. Kinderen van consanguine ouders hebben vooral een verhoogde kans op zeer zeldzame autosomaal recessieve aandoeningen. Deze groep zeldzame aandoeningen maakt een zeer klein deel uit van het totale aantal aangeboren aandoeningen. Een (beperkt) deel van deze aandoeningen leidt tot sterfte, het risico hierop is het grootst in het eerste levensjaar. In absolute aantallen is het aandeel van met consanguiniteit samenhangende aandoeningen in de perinatale en zuigelingensterfte zeer gering. Over de ziektelast door autosomaal recessieve aandoeningen, wanneer geen sprake is van sterfte, zijn geen goede cijfers beschikbaar. Er zijn geen goede representatieve cijfers over het voorkomen van consanguiniteit in Nederland. Uit incidentele en soms indirecte gegevens valt af te leiden dat huwelijken tussen verwanten in Nederland waarschijnlijk het meest frequent voorkomen in de Turkse en Marokkaanse bevolkingsgroepen en in kleine religieuze gemeenschappen. Hoewel de absolute aantallen klein zijn, hebben de betreffende aandoeningen een grote impact op de getroffen families. De aandacht voor tijdige opsporing en advisering over genetische risico's, waaronder consanguiniteit en het risico binnen specifieke bevolkingsgroepen op minder zeldzame autosomaal recessieve aandoeningen (zoals erfelijke bloedarmoede), neemt toe. Idealiter zou ieder aanstaand ouderpaar, bij voorkeur vssr een zwangerschap, zich moeten kunnen laten informeren over een eventueel verhoogde kans op een kind met een aangeboren aandoening en over mogelijkheden voor preventie.
