English Abstract
Paternal and maternal occupational exposure to chemicals may adversely affect human fertility and/or the development of the offspring of exposed individuals. To date, however, the data available are insufficient to enable all of the consequences of such work-related exposures to be quantified. In order to obtain an indication of the significance of these consequences, the National Institute for Public Health and the Environment (RIVM) has estimated the burden of disease of five clinical outcomes (such as spontaneous abortion and congenital abnormalities) and four groups of substances (such as pesticides and solvents). Taken together, these factors account for approximately 400 disability-adjusted life years (DALYs), which is 1% of the total amount of DALYs associated with reproductive effects among the working population (regardless of cause). Due to a lack of data and the uncertainty in various assumptions, the uncertainty margin of this estimate is quite large. The burden of disease is a concept used to quantify the consequences of illnesses relative to a disease-free situation. It is expressed in DALYs, a health gap measure which combines the time lived with disability (decreased quality of life) and the time lost due to untimely death. Many experts actively participated in this project as consultants on the delineations, points of departure, assumptions and the methods of calculation. Opinions varied greatly on a number of topics, such as whether the burden of disease of a spontaneous abortion only applies to the woman or also to her partner and/or unborn child. Only a few experts gave their opinion on the magnitude of total burden of disease of reproductive effects caused by chemical exposure in the workplace. These estimates varied between 100 and 10,000 DALYs. This range falls approximately in the middle of the range of the burdens of disease of nine other disorders caused by chemical exposure in the workplace that have previously been reported by RIVM.

Contact met chemicalien op de werkvloer kan gevolgen hebben voor de vruchtbaarheid van mannen en vrouwen of voor de ontwikkeling van het nageslacht. Er zijn echter te weinig gegevens beschikbaar om alle gevolgen van dergelijke arbeidsgerelateerde blootstellingen te kwantificeren. Om daar toch een indicatie van te krijgen, heeft het RIVM de ziektelast van vijf gevolgen (waaronder miskramen en aangeboren afwijkingen) en vier stofgroepen (zoals bestrijdings- en oplosmiddelen) geschat. Bij elkaar opgeteld bedraagt dit ongeveer 400 Disability Adjusted Life Years (DALY's). Dat is 1 procent van het totaal aantal DALY's voor effecten op de voortplanting onder de werkende bevolking (ongeacht de oorzaak). De onzekerheidsmarge van deze schatting is groot doordat beschikbare gegevens beperkt zijn en aannames in de berekeningen onzeker zijn. Het begrip ziektelast is een maatstaf om de gevolgen van ziekten en aandoeningen te kwantificeren, en wordt uitgedrukt in DALY's. Het combineert gezondheidsverlies door verminderde kwaliteit van leven en door vroegtijdig overlijden. Bij het onderzoek waren veel deskundigen betrokken. Zij zijn geraadpleegd over de afbakening, uitgangspunten, aannames en berekeningsmethode. Hun meningen liepen sterk uiteen. Bijvoorbeeld of de ziektelast van een miskraam alleen geldt voor de vrouw, of ook voor het ongeboren kind of de man. Slechts een klein aantal deskundigen deed een uitspraak over de verwachte totale jaarlijkse ziektelast voor effecten op de voortplanting door beroepsmatige blootstelling aan stoffen. Hun schattingen liepen uiteen van 100 tot 10.000 DALY's. Deze ziektelast neemt daarmee een middenpositie in ten opzichte van de totale ziektelasten van negen andere aandoeningen door beroepsmatige blootstelling aan stoffen, waarover het RIVM eerder heeft gerapporteerd.
