English Abstract
In a previous RIVM report the burden of disease due to exposure to chemicals at the workplace was estimated for nine investigated diseases. This follow-up study investigates if this burden of disease is mainly caused by exposure in the past or by exposure that is more recent. For diseases which are mainly or partly caused by exposure to chemicals in the past, it is investigated if the exposure at the workplace to several chemicals (relevant for the diseases in concern) has changed over time. Based on these developments and with some prudence the future trend in burden of disease can be predicted. However, these predictions contain a great deal of uncertainty. For asthma and contact dermatitis, it is impossible to predict the future trend in burden of disease because many different chemicals and several factors, partly working in the opposite direction, influence the development of these diseases. In the near future, the burden of disease of COPD and rhinitis and rhinosinusitis due to exposure to chemicals in occupational settings is not expected to change. For cardiovascular disorders, chronic toxic encephalopathy, skin- and lung cancer the future burden of disease of (due to exposure to chemicals in occupational settings) might be expected to decline. In the distant future, the burden of disease due to asbestos-exposure (mesothelioma, asbestos-related lung cancer and asbestosis) will be very small, but in the next thirty years still a large number of deaths has to be expected due to asbestos exposures in the past.

Gezondheidseffecten en ziektelast ten gevolge van blootstelling aan stoffen op de werkplek zijn voor een belangrijk deel terug te voeren op blootstellingen in het verleden. Aan de hand van gegevens over veranderingen in arbeidsgerelateerde blootstelling zijn daarom voorzichtige voorspellingen mogelijk over de toekomstige ziektelast voor bepaalde aandoeningen. In een eerder RIVM-rapport werd voor negen aandoeningen de ziektelast als gevolg van blootstelling aan stoffen op de werkplek geschat. In dit vervolgonderzoek is bekeken in hoeverre deze ziektelast wordt veroorzaakt door blootstellingen in het verleden of door meer recente blootstellingen. Wanneer de ziektelast voor een bepaalde aandoening in belangrijke mate wordt veroorzaakt door blootstellingen in het verleden, is bovendien onderzocht of de blootstelling aan enkele relevante stoffen op de werkplek in de loop van de tijd is veranderd. Op basis van de bevindingen luidt de voorzichtige voorspelling dat de ziektelast door blootstelling aan stoffen op de werkplek in de nabije toekomst zal afnemen voor hart- en vaatziekten, chronische toxische encefalopathie en huid- en longkanker. Voor andere aandoeningen blijft de ziektelast waarschijnlijk stabiel (COPD, rhinitis en rhinosinitis), of neemt die toe (mesothelioom, asbestlongkanker, asbestose). Voor astma en contact-eczeem is geen voorspelling mogelijk.
