English Abstract
The National Poisons Information Centre (NVIC) of the National Institute for Public Health and the Environment (RIVM) provides information by telephone, 24 hours a day, on possible effects and treatment of health-threatening complaints caused by exposure to chemical substances. The NVIC functions in the Netherlands as the central point of contact for (potentially) acute intoxications; it is consulted frequently by physicians and other relief workers such as the police and the fire brigade. The annual increase in the number of enquiries continued in 2003, increasing from nearly 32,000 consultations in 2002 to more than 33,000 in 2003, concerning 43,948 different exposures to chemical substances. This survey was compiled to provide insight into the acute intoxications in the Netherlands showing the highest incidence. Following the introduction, chapter 2 of the report describes the role of the NVIC in noting new developments and trends in acute intoxications. The (consumer) products that were frequently involved in acute intoxications and the substances that caused potentially serious health effects are indicated. Chapter 3 goes more deeply into the role of the NVIC in calamities and describes a number of incidents that took place in 2003, for illustration. It also discusses the electronic differential diagnostic system for chemical substances, the latest product developed by the NVIC. This system serves to interpret a patient's complaints and symptoms as soon as possible and to identify the causative agents. Overdosing with medicines occurred frequently in 2003, with the NVIC being consulted for 23,163 cases of exposure to medication. The number of enquiries about sleep-inducing drugs, anxiolytics, antidepressants and antipsychotics increased once again. The same goes for the painkillers paracetamol and ibuprofen. In terms of percentage, the largest increase for children aged 0 to 12 was seen for the multivitamins, with an increase of 25% compared to 2002. The tendency to make packages and vitamin products look like sweets is an undesirable development from the viewpoint of intoxication prevention. Likewise, more and more non-food (cosmetic) products have packaging or an appearance that resembles drinks or foodstuffs. Since consumers, especially children, can confuse these products with genuine foodstuffs, they are more inclined to put them in their mouths, suck on them or swallow them, thus increasing the risk of intoxication. Mistaking denture-cleaning tablets for peppermints, has been a cause of accidental ingestion of these tablets for years in the very old. Small children were most frequently involved in (potential) intoxication with household products (especially cleaning and petroleum-containing products). During, and shortly after, the large public information campaign "Toxic Seducers" by the Consumer Safety Institute from September 2000 to the spring of 2002, the number of intoxications caused by these products in children appeared to decrease. However, in 2003 exposures had increased again by 19% compared to 2002. Informing parents about the dangers of products in their homes thus remains necessary. The number of times the NVIC was consulted in 2003 about acute alcohol intoxication in young people between 13 and 17 increased by 63% compared to 2002. The total number of enquiries about drug intoxications has risen considerably too, almost doubling during the past four years, from 656 enquiries in 2000 to 1210 enquiries in 2003. The highest increases were found in the smart-product group, containing "energizers" such as ginseng and, especially, ephedra. After showing a sharp increase in 2000 and 2001, the number of intoxications with GHB (gamma hydroxybutyric acid) has now more-or-less stabilised.

Het Nationaal Vergiftigingen Informatie Centrum (NVIC) van het Rijksinstituut voor Volksgezondheid en Milieu (RIVM) verstrekt 24 uur per dag telefonische informatie over de mogelijke gevolgen en behandeling van patienten met gezondheidsbedreigende aandoeningen als gevolg van blootstelling aan chemische stoffen. Het NVIC is in Nederland het centrale aanspreekpunt in het geval van (potentiele) acute vergiftigingen en wordt veelvuldig geconsulteerd door medici en andere hulpverleners zoals politie en brandweer. De jaarlijkse stijging van het aantal informatieverzoeken zette zich door in 2003; de vraag steeg van bijna 32.000 consulten in 2002 naar ruim 33.000 in 2003, over in totaal 43.948 verschillende blootstellingen aan chemische verbindingen. Dit overzicht is samengesteld om inzicht te geven in de in Nederland meest voorkomende acute vergiftigingen. In hoofdstuk 2 komt de signaleringsfunctie van het NVIC aan bod en wordt aangegeven met welke (consumenten) producten veel intoxicaties voorkwamen en welke stoffen vooral tot potentieel ernstige gezondheidsproblemen aanleiding gaven. In hoofdstuk 3 wordt nader ingegaan op de rol van het NVIC bij calamiteiten en worden ter illustratie een aantal incidenten uit 2003 besproken. Tevens wordt hier het nieuwste door het NVIC ontwikkelde product besproken: een digitaal differentiaal diagnostisch systeem voor chemische stoffen. Dit systeem dient om zo snel mogelijk klachten en symptomen van patienten te kunnen duiden en de veroorzakende stoffen te identificeren. Overdoseringen met geneesmiddelen kwamen veel voor: het NVIC werd over 23.163 blootstellingen aan geneesmiddelen geraadpleegd. Het aantal informatieverzoeken over slaapmiddelen, anxiolytica, antidepressiva en antipsychotica is weer verder toegenomen. Datzelfde geldt voor de pijnstillers paracetamol en ibuprofen. Procentueel de grootste stijger bij de kinderen t/m 12 jaar waren de innames van multivitaminen, met een toename van 25% ten opzichte van 2002. De trend om verpakkingen en vitaminepreparaten op snoep te laten lijken is vanuit het oogpunt van preventie van vergiftigingen zeer ongewenst. Evenzeer geldt dat steeds meer niet-eetbare (cosmetica)producten een voorkomen of verpakking hebben die lijkt op eet- of drinkwaren. Ook deze producten verhogen de kans op vergiftigingen, doordat consumenten, met name kinderen, ze kunnen verwarren met echte voedingsmiddelen en sneller geneigd zijn ze in de mond te steken, op te zuigen of in te slikken. Het aanzien van kunstgebitreinigingstabletten voor pepermuntjes, is bij hoogbejaarden al jaren een oorzaak van accidentele inname van deze tabletten. Kleine kinderen waren het meest frequent betrokken bij (potentiele) vergiftigingen met huishoudmiddelen (vooral schoonmaakmiddelen en petroleumproducten). Tijdens en kort na de grote publieksvoorlichtingscampagne "Giftige Verleiders" van de Stichting Consument en Veiligheid van september 2000 tot voorjaar 2002 leek er sprake te zijn van een daling in het aantal kinderintoxicaties met deze middelen, echter in 2003 is het aantal blootstellingen weer met 19% gestegen ten opzichte van 2002. Voorlichting aan ouders over de gevaren van in huis aanwezige producten blijft noodzakelijk. Het aantal keren dat het NVIC in 2003 is geraadpleegd over acute alcoholintoxicaties bij jongeren van 13 t/m 17 jaar is 63% gestegen ten opzichte van 2002. Het totaal aantal informatieverzoeken over intoxicaties met drugs is eveneens flink toegenomen, met in de laatste 4 jaar bijna een verdubbeling van 656 meldingen in 2000 naar 1210 in 2003. De sterkste stijgers waren te vinden in de groep van de smartproducts, met "energizers" als ginseng en met name efedra. Het aantal intoxicaties met GHB (gammahydroxyboterzuur) heeft zich, na een sterke toename in 2000 en 2001, redelijk gestabiliseerd.
