English Abstract The state of the art and developments expected in the
future telemedicine and telecare applications in home care form a component
of the Public Health Status and Forecast 2002, published by the National
Institute for Public Health and the Environment in the Netherlands. Major
technological advances, represented here as telemedicine and telecare (the
information and telecommunication technology (ICT) applications for health
care) have opened up a wide array of possibilities for the care sector.
However, except for social alarm systems and Internet as a means of
acquiring information, there have, to date, been few successful practical
initiatives in telemedicine and telecare. Insufficient technical
infrastructure and a lack of scientific data on the cost-effectiveness of
telemedicine and telecare may be potential obstacles. The current
organisation of health care and resistance to change in the relevant
organisations may also hamper further implementation. The key to a wider
implementation of telemedicine and telecare is providing stakeholders (i.e.
insurers, health-care professionals, patients and government) with robust
data on the practical implications of initiatives. This can be followed by
assessment to see if claims that telemedicine and telecare combine a high
quality of care with lower costs and a lower human resources input are
justifiable. The expected higher demand for care in the Netherlands due to
an ageing population, a relatively small active workforce, an increase in
the number of people with a chronic illness and a tendency among patients to
ask for more (personal) care form the motive behind the current interest in
telemedicine and telecare.
Rapport in het kort
Dit rapport naar de stand van zaken en toekomstige
medisch-technologische ontwikkelingen van telemedicine en telecare in de
thuiszorg vormt een onderdeel van de Volksgezondheid Toekomst Verkenning
2002 van het Rijksinstituut voor Volksgezondheid en Milieu. De gegevens
zijn verkregen door middel van literatuuronderzoek. Het toenemende aantal
personen met een chronische ziekte, het aantal hoogbejaarden, het
personeelstekort, een veeleisender patient en de toenemende kosten stellen
de zorgsector voor nieuwe uitdagingen. Het aandeel van de werkende ten
opzichte van de niet-werkende bevolking wordt daarbij steeds kleiner.
Telemedicine en telecare omvatten 'zorg op afstand' met behulp van
informatie- en communicatietechnologie (ICT). Deze vorm van ondersteuning
van zorg kan mogelijk een positieve bijdrage leveren aan de efficientie en
doelmatigheid in de zorg, vanzelfsprekend met behoud van de kwaliteit,
waardoor ontstane knelpunten kunnen worden opgelost. Er is een aantal
kansrijke toepassingen van telemedicine en telecare op een breed terrein.
Deze toepassingen zijn: sociale alarmering, zorgcoordinatie,
kennisoverdracht, telemonitoring, teleconsultatie, televisites, instructie/
begeleiding/ training op afstand en preventie. Op het gebied van sociale
alarmering zijn al successen behaald. Ook het gebruik van Internet voor het
verkrijgen van gezondheidsinformatie is inmiddels wijdverbreid. De overige
toepassingen staan veelal nog in de kinderschoenen. Het succes van
initiatieven wordt beperkt door met name vraagtekens omtrent de
kosten-effectiviteit, de manier van organisatie en de weerbarstigheid van
bestaande culturen in de zorg. De verantwoording voor het uitvoeren van
(kosten-effectiviteit)onderzoek ligt bij alle partijen, al lijkt regie van
de overheid bij grotere (ook de thuiszorg overstijgende) projecten zoals het
Elektronisch Patienten Dossier gewenst.
