English Abstract
There is a need for local authorities and emergency response services to estimate in advance both the number of people that may be injured due to accidents with chemical substances and the nature of the related injuries. This information, as well as the number of possible casualties, is necessary for authorities to be able to assess both spatial planning and applications related to activities requiring (local) government permits. The National Institute for Public Health and the Environment (RIVM) has drawn up a proposal for such an operating system. Methods already exist for calculating the number of casualties of chemical accidents, but not for the number of people suffering injuries. The proposal of the RIVM for calculating the number of injured is a rough draft only; as such, it still requires further refining in terms of developing procedures, obtaining data and establishing criteria. The proposed system consists of three phases. In the first phase, local authorities and emergency response services define the activities and scenarios - for example, those associated with a possible incident involving the exposure of people to chemicals and the consequences thereof. In the second phase, emergency response services calculate the number of individuals that may be injured, and which of these may need medical help, as well as the kind of injuries that may occur. The estimates arrived at independently by the emergency response services in the second phase are supported by established data and procedures and by effect-concentrations based on de AEGL (Acute Exposure Guideline Level) data. One possible result of these calculations is that the number of injured exceeds the capacity of both the emergency response services and local hospitals. In this case, the choice to be made by the (local) authorities is either to modify the activity (not allow it or allow it only under specific regulatory control) or to carry out an in-depth assessment of the activity by bringing in experts (third phase). These experts will provide a more precise estimate of number of the injured and types of injuries based on their experience, exposure levels and detailed toxicological data.

Bestuurders en lokale hulpverleners hebben er behoefte aan om van tevoren het aantal gewonden in te schatten als gevolg van mogelijke incidenten met chemische stoffen, evenals de aard van de verwondingen. Deze informatie is, naast gegevens over het mogelijke aantal dodelijke slachtoffers, nodig om ruimtelijke ordeningsplannen en aanvragen voor vergunningsplichtige activiteiten te kunnen beoordelen. Het RIVM doet een voorstel voor een werkwijze hiervoor. Er bestaan al methoden om het aantal dodelijke slachtoffers te berekenen, maar nog niet om het aantal gewonden in te schatten. Het RIVM geeft met dit voorstel een schets van een systeem dat verder in protocollen, gegevens en criteria uitgewerkt en beschikbaar moet komen. De voorgestelde werkwijze omvat drie fasen. In de eerste fase kunnen bestuurders en hulpverleners zelf de activiteit definieren, evenals het mogelijke incident en de blootstelling aan stoffen als gevolg daarvan. In de tweede fase kunnen hulpverleningsdiensten - ook zelf - het aantal mensen berekenen dat niet in staat is zichzelf te redden en het aantal van hen dat hulpverlening nodig heeft. De inschatting van het aantal gewonden is in deze fase gebaseerd op de zogeheten rampeninterventiewaarden. Daaruit kan blijken dat het aantal gewonden te hoog is ten opzichte van de geschatte benodigde capaciteit van de hulpverlening. De keus is dan de activiteit aan te passen (dat wil zeggen, niet of onder voorwaarden toestaan) of een nadere analyse te laten uitvoeren. Voor die analyse wordt deskundige expertise ingezet (derde fase). Aan de hand van de blootstelling aan een stof wordt dan een nauwkeurigere inschatting van het aantal slachtoffers berekend.
