English Abstract
Since the decline in the number of drinking-water installations with collective pipe networks that contain Legionella is not marked, no effect of the regulations on the prevention of Legionella can be determined. This was shown in a RIVM investigation performed under the authority of the Dutch Ministry for Housing, Spatial Planning and the Environment (VROM). Between 2000 and 2005 the number of patients with Legionnaire's disease increased from 172 to 280. In 2006 this number rose to as high as 440 patients, and in the same year, the mortality of patients infected within the Netherlands exceeded the maximum permissible mortality of 1 per million inhabitants. Noteworthy here is that only for 3% of the infections (19 patients) could the source be identified with confidence. Half of these 19 patients were infected through water from a drinking-water installation supplying a hospital. If the regulations with respect to the prevention of Legionella had been better observed, this would probably have led to fewer drinking-water installations containing Legionella, made possible by having fitters install drinking-water installations meeting the requirements. This also holds for owners who should have implemented the regulations on the prevention of Legionella. Currently, this is not the case. This five policy options described vary from continuation of the existing regulations to a policy that focuses on the elimination of Legionella pneumophila only. Especially for the second option it is important to improve the implementation of the regulations. For example, closure of showers in a swimming pool or complete hospital wards could not be necessary if only the non-hazardous Legionella non-pneumophila is detected. Recommendations are given for fitters, water-supply companies, policy-makers and regional inspectors from VROM, as well as recommendations for follow-up research. Attention should also be paid to other potential infection sources that produce aerosols, including cooling towers.

Het aantal leidingwaterinstallaties met legionellabacterien neemt niet duidelijk af. Daardoor is nog geen effect vast te stellen van het beleid ter preventie van Legionella in leidingwaterinstallaties. Dit blijkt uit onderzoek van RIVM in opdracht van het Ministerie van VROM. Het aantal legionellosepatienten in Nederland nam tussen 2000 en 2005 toe van 172 naar 280. In 2006 lag het aantal zelfs op 440. De sterfte onder in Nederland geinfecteerde patienten was dat jaar hoger dan de maximaal aanvaardbare sterfte van 1 op de miljoen inwoners. Hierbij moet worden aangetekend dat in de afgelopen vier jaar slechts bij circa 3 procent (19 patienten) de besmettingsbron met zekerheid is vastgesteld. De helft van de 19 patienten werd geonfecteerd via een leidingwaterinstallatie in een zorginstelling. Naar verwachting leidt de regelgeving wel tot minder legionellabacterien in leidingwaterinstallaties als de naleving ervan wordt verbeterd. Dit kan als installateurs consequent leidingwaterinstallaties aanleggen volgens de voorschriften en de eigenaren de legionellapreventiemaatregelen uitvoeren. Dit gebeurt nu te weinig. Dit rapport beschrijft vijf beleidsopties, varierend van het handhaven van het bestaande legionellapre-ventiebeleid tot beleid dat zich uitsluitend richt op de gevaarlijke variant Legionella pneumophila. Vooral voor deze laatste optie is het belangrijk dat er verbetering komt in de naleving en controle van de preventiemaatregelen. Uit de gekozen beleidsoptie volgt of maatregelen, zoals het afsluiten van zwembaddouches of gehele ziekenhuisafdelingen, nodig zijn indien alleen de ongevaarlijke variant Legionella non-pneumophila wordt aangetroffen. Tevens bevat dit rapport aanbevelingen voor leidingwaterinstallateurs, waterbedrijven, VROM-Inspectie en VROM-beleid, en tips voor vervolgonderzoek. Er moet meer worden gelet op andere be-smettingsbronnen met verneveling, waaronder koeltorens.
