Samenvatting

In 2003 werd in Nederland 57,5 miljard euro aan de gezondheidszorg uitgegeven, hetgeen overeenkomt met ongeveer 12% van het bruto binnenlands product. Per inwoner ging het om een bedrag van 3.550 euro. Dit rapport beschrijft hoe deze zorgkosten samenhangen met kenmerken van zorggebruikers als ziekte (diagnosegroep), leeftijd en geslacht, het aanbod van zorg (sectoren en zorgfuncties) en de financiering. Van de totale zorgkosten kon 82,5% aan een ziekte worden toegewezen. Psychische stoornissen zijn met 12,7 miljard euro de duurste diagnosegroep. Hierin zijn ook de kosten van verstandelijke handicaps en dementie opgenomen. Op afstand volgen ziekten van het hartvaatstelsel (5,3 miljard euro) en ziekten van het spijsverteringsstelsel (4,6 miljard euro). Symptomen en onvolledig omschreven ziektebeelden (4,2 miljard euro) en aandoeningen van het bewegingsstelsel en bindweefsel (3,9 miljard euro) completeren de top-5. De zorgkosten hangen sterk samen met de leeftijd, en lopen op tot gemiddeld meer dan 40.000 euro per inwoner van Nederland in de hoogste leeftijdsgroepen. Het aandeel van mannen in de totale zorgkosten van 2003 bedroeg 42% en van vrouwen 58%. Het verschil wordt veroorzaakt door de kosten van zwangerschap en geboorte en vooral ook door de hogere levensverwachting van vrouwen.het CBS. Naast dit perspectief zijn nog twee andere invalshoeken volledig uitgewerkt, namelijk het het Budgettair Kader Zorg (BKZ) van het ministerie van VWS en het System of Health Accounts (SHA) van de OECD.

Abstract

The Netherlands spent 57.5 billion euro on health care in 2003, an equivalent to about 12% of the Gross National Product (GNP) or 3,550 euro per capita. In this report health expenditure is described by aspects of healthcare demand (disease, age and gender), supply (providers and functions) and funding.
The treatment of particular diseases involved 82.5% of the total healthcare costs. Mental illnesses represent the most expensive diagnosis group (12.7 billion euro) including costs for the mentally retarded and those afflicted by dementia. The costs for heart disease amounted to 5.3 billion euro and for diseases of the digestive system to 4.6 billion euro. The costs for symptomatic complaints and incompletely described syndromes (4.2 billion euro), along with disorders of the musculoskeletal system (3.9 billion euro) complete the list of the top five most expensive disease groups.
Healthcare costs depend heavily on age, and amounted in 2003 up to about 40,000 euro per inhabitant in the highest age groups. Men participated in healthcare costs with 42% and women with 58%. This is partly due to the extra costs related to pregnancy and childbirth, but meanly results from the higher life expectancy of women. The above-mentioned costs are based on the Dutch Health and Social Care Accounts. Because other perspectives on health expenditure are important as well, all costs are also specified according the Healthcare Budget Framework (BKZ) of the Dutch Ministry of Health and the OECD's System of Health Accounts (SHA).
