English Abstract
The RIVM has executed an exploratory study on recent developments and risks in the field of medical technology in the Netherlands in Dutch hospitals. Results from this exploratory study show that there is relatively a lot of clinical research with new implants. An example of such a new implant is the wireless pacemaker, a small rod-shaped device that is implanted directly into the chamber of the heart. First experiences show that the placement can be tricky. Furthermore, it is not yet known whether the pacemaker can be easily removed after a period of time when the battery runs out and the device has to be replaced. These types of innovations are currently being used in limited groups of patients. Results from these clinical studies will have to prove whether these innovations are actually appropriate for broader application.

Furthermore, within the Dutch hospitals developments are seen in the field of imaging equipment, such as equipment for making 3D-echoes and cutting equipment for surgeons. Misuse of medical equipment, for example, cutting equipment, can cause serious harm to the patient. However, even if such a new technology is being used correctly, there can be risks. This was recently demonstrated by an incident involving the use of a medical device for the removal of fibroids in the uterus; there is a risk that the procedure will spread the cancerous tissue within the abdomen and pelvis. Therefore, it is important that medical technologies are carefully introduced into the hospital so that users are fully aware of the capabilities and limitations of the device.

Het RIVM heeft een verkenning uitgevoerd naar de recente ontwikkelingen en de risico's op het gebied van medische technologie in de Nederlandse ziekenhuizen. Resultaten uit deze verkenning laten zien dat er relatief veel klinisch onderzoek is uitgevoerd met nieuwe implantaten. Een voorbeeld van zo'n nieuw implantaat is de draadloze pacemaker, een klein staafvormig apparaatje dat direct in de hartkamer wordt geïmplanteerd. Uit de eerste ervaringen blijkt dat de plaatsing soms lastig is. Verder is nog niet bekend of de pacemaker na verloop van tijd eenvoudig kan worden vervangen. Dat is in ieder geval na enkele jaren aan de orde als de batterij op is. Dergelijke innovaties worden momenteel nog maar bij een beperkte groep patiënten geplaatst. Resultaten uit deze klinische onderzoeken zullen nog moeten uitwijzen of deze innovatie daadwerkelijk breed toepasbaar is.

Verder blijkt dat er binnen de ziekenhuizen ontwikkelingen gaande zijn op het gebied van beeldvormende apparatuur, zoals apparatuur voor het maken van 3D-echo's en snijapparatuur voor chirurgen. Verkeerd gebruik van medische apparatuur, zoals snijapparatuur, kan ernstige schade bij de patiënt veroorzaken. Maar ook als een dergelijke nieuwe technologie correct wordt gebruikt, kunnen er risico's optreden. Dat bleek bijvoorbeeld recentelijk uit incidenten bij het gebruik van een medisch apparaat om vleesbomen uit de baarmoeder te verwijderen, waarbij aanwezige kwaadaardige kankercellen losgemaakt waren en niet konden worden verwijderd. Het is dus belangrijk dat medische technologieën zorgvuldig worden geïntroduceerd in de instelling, zodat de gebruiker goed op de hoogte is van de mogelijkheden en beperkingen van het apparaat.
