English Abstract
The Municipal Public Health Services (GGD) advise people on issues such as health problems arising from the physical living environment, due to the pollution of soil, air, water, or the indoor environment by chemical contaminants and radiation, for example. In the interests of standardisation, this advice is based on guidelines. The National Institute for Public Health and the Environment (RIVM) has set out a new vision for drawing up environmental health guidelines. The most important changes are that these guidelines are now developed systematically, using an "evidence based" approach, and that they can be continuously adjusted.

The "evidence-based" approach can range from expert opinion based on practical experience to the highest level of scientific evidence. The former is particularly important in situations where there is no scientific substantiation.

Guidelines are designed as "living documents", to make it possible to follow national and international developments and, where necessary, adjust the guideline accordingly. Guidelines are routinely evaluated and adjusted (if required) every five years, but it is now possible to make interim adjustments as well.

Guidelines are not binding. This procedure was designed to enable guidelines to be optimally developed and managed. Depending on the local situation, it is possible - and sometimes even necessary - to deviate from a guideline, provided that a reasoned argument is given for doing so.

This new vision, which was financed by the Ministry of Health, Welfare and Sport, was prepared jointly by RIVM and the GGDs. The detailed repercussions of the proposed changes will be identified by a working group consisting of representatives from the GGDs and RIVM. The aspects examined in this context include the feasibility of changes, the time required to implement them, and the demands this places on the organisations involved, in terms of commitment.

De GGD adviseert de bevolking onder meer bij gezondheidsproblemen die voortkomen uit de fysieke leefomgeving, zoals verontreinigingen van bodem, lucht en water of het binnenmilieu met onder andere chemische verontreinigingen en straling. Om de adviezen te uniformeren, worden ze ontleend aan richtlijnen. Het RIVM heeft een nieuwe visie beschreven om de richtlijnen medische milieukunde op te stellen. De belangrijkste veranderingen zijn dat ze systematisch op basis van het 'beste bewijs' worden ontwikkeld en dat ze de mogelijkheid bevatten om continu te worden aangepast.

Het 'beste bewijs' (evidence-based) kan variëren van het hoogste niveau van wetenschappelijk bewijs tot de mening van deskundigen op basis van praktijkervaringen. Dit laatste is relevant wanneer een wetenschappelijke onderbouwing ontbreekt.

Door de richtlijn als 'levend document' in te richten, wordt de mogelijkheid gecreëerd om nationale en internationale ontwikkelingen te volgen en de richtlijn daar zonodig op aan te passen. Richtlijnen worden in elk geval elke vijf jaar geëvalueerd en desgewenst aangepast, maar dat kan nu ook tussentijds.

Richtlijnen zijn niet bindend. De procedure is dusdanig opgesteld dat de richtlijnen optimaal worden ontwikkeld en beheerd. Het is mogelijk, en soms zelfs nodig, om, mits gemotiveerd, afhankelijk van de lokale situatie van een richtlijn af te wijken.

De nieuwe visie is door het RIVM samen met GGD'en opgesteld en is gefinancierd door het ministerie van VWS. De consequenties van de voorgestelde veranderingen zullen nog door een werkgroep van GGD'en en het RIVM worden uitgewerkt. Hierbij wordt onder andere gekeken naar de haalbaarheid van veranderingen, de termijn waarop ze gerealiseerd kunnen worden en de inzet die het van betrokken organisaties vergt.
