Verkenning gezondheidsrisico's ultrafijnstof luchtvaart rond Schiphol en voorstel vervolgonderzoek

English Abstract
There are no evident indications that mortality risks in residential areas near Schiphol airport deviate from risks in nearby regions or elsewhere in the Netherlands. This is the result of an explorative study from RIVM into the health risks of ultrafine particles from aviation around Schiphol airport.

The study was conducted by the order of the ministry of Infrastructure and the Environment. The task was to give, at short notice, insight into the risk of premature mortality in residential areas around the airport. In addition, the ministry requested an advice about the feasibility and design of a possible follow-up study.

Data on all-cause mortality and cause specific mortality for cardiovascular disease, respiratory disease and lung cancer from Statistics Netherlands was used for the study. The postal codes around the airport were compared with each other and with other regions of the Netherlands. Also it was explored whether a spatial pattern in the mortality risk was present in locations relative to the airport. Elevated as well as lower risks were observed, but there was no apparent clustering of risks in postal codes around the airport. No direct comparison was made between the exposure at the home address and the mortality risk.

In earlier studies from TNO and RIVM, elevated levels of ultrafine particles from aviation were found up to 15 km distance from the airport. There are indications that ultrafine particles in aircraft engine emissions could lead to health effects. Possible small deviations in mortality risks are difficult to detect with the methods applied in this explorative study.

Additional research is necessary to gain more accurate insight in the extend that ultrafine particles can contribute to health effects. This can be achieved by designing a research programme in which, besides mortality, several health effects are considered, like cardiovascular and respiratory disorders and birth weight. Partly, these studies can be carried out retrospectively using available data from health registries. It is also recommended to measure ultrafine particles for a few years in the vicinity of the airport, since the levels reported in the earlier studies have a substantial degree of uncertainty. The proposed research programme may supply more clarity about the consequences of exposure to ultrafine particles.

Er zijn geen duidelijke aanwijzingen dat de sterfterisico's nabij de luchthaven Schiphol afwijken van die van nabijgelegen gebieden of elders in Nederland. Dit blijkt uit een eerste verkennend onderzoek van het RIVM naar de gezondheidsrisico's van ultrafijnstof rond Schiphol door de luchtvaart.

Het onderzoek is uitgevoerd in opdracht van het ministerie van Infrastructuur en Milieu (IenM). De opdracht was om op korte termijn inzicht te geven in het risico om vroegtijdig te overlijden in de woongebieden rondom Schiphol. Daarnaast wil het ministerie advies over de haalbaarheid en opzet van een eventueel vervolgonderzoek.

Voor deze eerste verkenning zijn sterftecijfers van het CBS gebruikt van 2004 tot en met 2010, zowel voor algemene sterfte als sterfte aan specifieke aandoeningen aan de luchtwegen, het hart- en vaatstelsel en longkanker. De postcodegebieden rond Schiphol zijn met elkaar vergeleken en met andere delen van Nederland. Ook is gekeken of er een patroon kon worden gevonden in sterftecijfers in relatie tot de afstand tot de luchthaven, of tot gebieden met verschillende concentraties ultrafijnstof. Hierbij zijn zowel hogere als lagere sterftecijfers gevonden en er was geen duidelijke clustering te zien van een hoger sterfterisico rond Schiphol. Het was niet mogelijk om een vergelijking te maken tussen de sterftecijfers en de blootstelling aan ultrafijnstof op het woonadres.

Uit eerder onderzoek van TNO en het RIVM uit 2015 blijkt dat de concentratie ultrafijnstof tot op 15 km van de luchthaven door het vliegverkeer wordt verhoogd. Er zijn redenen om aan te nemen dat deze ultrafijnstofdeeltjes uit vliegtuigmotoren gezondheidseffecten kunnen veroorzaken. Eventuele kleine verhogingen in sterftecijfers zijn met de gebruikte onderzoekstechnieken moeilijk terug te zien in de bevolkingsgroep die dit aangaat.

Aanvullend onderzoek is nodig om beter inzicht te krijgen in de mate waarin ultrafijnstof bijdraagt aan gezondheidseffecten. Dit kan worden bereikt door een integraal onderzoeksprogramma op te zetten. Daarin moet niet alleen naar sterfte worden gekeken, maar ook naar verschillende andere gezondheidsaspecten, zoals aandoeningen van de luchtwegen en het hart- en vaatsysteem, medicijngebruik en geboortegewicht. Dit kan voor een deel met terugwerkende kracht op basis van beschikbare gezondheidsgegevens. Het is ook raadzaam om een aantal jaren de concentraties ultrafijnstof in de directe omgeving van Schiphol te meten, omdat de concentraties uit de eerdere onderzoeken nog onzekerheid kennen. Het voorgestelde vervolgonderzoek kan hier meer duidelijkheid over verschaffen.
