Every year, RIVM analyses incidents involving hazardous substances at large chemical companies. The 2017-2018 analysis concerns twelve incidents, including a fire and two explosions. The other incidents involved the release of hazardous substances. Three people were injured. Their injuries were such that they would probably recover from them.

Chemical companies must ensure that their installations are in order and that their production processes and activities are carried out safely. In the twelve incidents analysed, problems arose with various elements, for example because materials had weakened or chemical reactions were not properly controlled. The resulting deviations were not promptly discovered and remedied because of inadequate monitoring and inspection. Six incidents could have been avoided if an appropriate emergency protection measure had been in place. These emergency protection measures, such as protecting the installation against overpressure, had not been adequately implemented, if at all, or had not been properly maintained.

In nine incidents, the underlying working procedures were not properly implemented or had not been properly applied. Furthermore, personnel were sometimes not competent and alert enough and sometimes they were unable to work safely because of insufficient suitable materials.

This annual report is part of a multi-year order from the Ministry of Social Affairs and Employment (SZW) to analyse accidents investigated by the Major Hazard Control (MHC) Directorate of the SZW Inspectorate. RIVM examines the similarities and differences between these incidents. These findings can be used for inspection and enforcement strategies and companies can use the insights to improve their safety policies.

Het RIVM analyseert jaarlijks incidenten met gevaarlijke stoffen bij grote chemische bedrijven. De analyse van 2017-2018 omvat twaalf incidenten, waaronder een brand en twee explosies. Bij de overige incidenten kwamen gevaarlijke stoffen vrij. Drie mensen raakten gewond. Hun letsel was vermoedelijk van herstelbare aard.

Chemische bedrijven moeten zorgen dat installaties op orde zijn en dat productieprocessen en werkzaamheden veilig worden uitgevoerd. Bij de twaalf incidenten ging het op verschillende onderdelen mis, bijvoorbeeld doordat materialen verzwakten of chemische reacties niet goed werden beheerst. De afwijkingen die daarbij ontstonden, werden door gebrek aan controle en inspectie niet tijdig ontdekt en hersteld. Bij zes incidenten had een noodmaatregel het incident nog kunnen voorkomen. Deze noodmaatregelen, zoals de installatie beschermen tegen overdruk, waren echter niet of niet adequaat geïmplementeerd of niet goed onderhouden.

Bij negen incidenten waren achterliggende werkprocedures niet goed op orde of werden ze niet goed uitgevoerd. Daarnaast was de competentie en alertheid van het personeel soms ontoereikend en waren er soms onvoldoende geschikte materialen om veilig te kunnen werken.

Deze jaarlijkse rapportage maakt deel uit van een meerjarige opdracht van het ministerie van Sociale Zaken en Werkgelegenheid (SZW) om incidenten te analyseren die door de Inspectie SZW zijn onderzocht. RIVM gaat na wat de overeenkomsten en verschillen tussen deze incidenten zijn. De bevindingen kunnen gebruikt worden voor de inspectie- en handhavingsstrategieën. Bedrijven kunnen de inzichten gebruiken om hun veiligheidsbeleid te verbeteren.
