Antibiotic resistance is becoming an increasing global threat to public health. Over-use and improper use of antibiotics are important drivers of rising resistance. Dutch physicians generally prescribe less antibiotics compared to physicians in other countries, but improvements can still be made by prescribing antibiotics only when really needed.

The Dutch national action plan on antibiotic resistance, initiated in 2015, set a goal of reducing improper prescription of antibiotics by 50 percent. This requires that we know how often and when physicians prescribe antibiotics, and when prescriptions can be said to be improper. RIVM, in collaboration with the academic workplace for geriatric care in Amsterdam (UNO-VUmc), developed a tool for this purpose, specifically for geriatric care specialists.

The tool uses minimal registration in the electronic patient records to provide insight into the prescribing behaviour of physicians working in long term care. These geriatric care specialists work with the elderly living in long term care facilities and people receiving long term care for other reasons. These persons are generally more often prescribed antibiotics for lower respiratory tract infections or urinary tract infections.

The physicians participated in a pilot study where they tracked their prescribing for these two types of infections for three months. RIVM and UNO-VUmc collected and analyzed the data and presented the outcomes to the physicians. This 'data reflection' raises awareness of prescribing behaviour among physicians and allows them to improve. All participating physicians found this information to be very useful and expressed interest in repeating the process.
This tool was developed in a previous pilot and has now been improved. Participating physicians found the tool easy and quick to use. Two newly confirmed guidelines for treatment of lower respiratory tract and urinary tract infections in the elderly were also included. The tool was found to be very helpful in implementing the new guidelines.

Resistentie tegen antibiotica is wereldwijd een steeds grotere bedreiging voor de volksgezondheid. Te veel en onnodig gebruik van antibiotica zijn belangrijke oorzaken van die stijging. Nederlandse artsen schrijven in het algemeen niet snel antibiotica voor. Toch is er nog winst te halen als zij het alleen geven als het echt nodig is.

De landelijke aanpak antibioticaresistentie uit 2015 wil 'onjuist' antibioticagebruik met 50 procent verminderen. Hiervoor is het belangrijk te weten hoe vaak en in welke situaties artsen antibiotica voorschrijven aan ouderen, en hoe vaak dit als onjuist aangeduid kan worden. Het RIVM heeft hiervoor met de academische werkplaats ouderenzorg van Amsterdam UMC (UNO-VUmc) een methode ontwikkeld.

De methode geeft met een minimale registratie in het elektronisch cliëntendossier inzicht in het voorschrijfgedrag van artsen die werken in de langdurige zorg. Het gaat om artsen die werken met ouderen in verpleeghuizen en kwetsbare personen die lange tijd zorg nodig hebben. Deze mensen krijgen relatief vaak antibiotica voor infecties aan de lage luchtwegen en urinewegen.

De artsen hebben als proef drie maanden bijgehouden hoe vaak en wanneer zij antibiotica voorschreven voor deze twee infecties. Het RIVM en UNO-VUmc hebben de gegevens verzameld, geanalyseerd en de resultaten aan de artsen gegeven. Met deze 'spiegelinformatie' worden artsen bewuster van hun voorschrijfgedrag en kunnen zij dit zo mogelijk verbeteren. De artsen vonden deze informatie heel nuttig. Alle deelnemers willen de registraties in de toekomst herhalen.

De methode was in een eerder proefproject ontwikkeld en is nu verbeterd. De artsen vonden de methode nu veel gebruiksvriendelijker. Ook zijn er twee nieuwe richtlijnen in verwerkt om de genoemde mensen te behandelen voor infecties aan de lage luchtwegen en infecties aan de urinewegen. De methode blijkt een hulpmiddel voor artsen om goed volgens de twee nieuwe richtlijnen te handelen.
