English Abstract
Objective: To determine the seroprevalence of hepatitis-B viral (HBV) markers in the general Dutch population. Methods: A serum bank with sera from a representative sample of the Dutch population (0-79 years) was established between October 1995 and December 1996 by means of a cross-sectional population-based study. The presence of anti-HBc was assayed in 7395 sera and those found positive were further tested for HBsAg. Results: The seroprevalence of anti-HBc as a marker of current or past infection in the general Dutch population was 2.1%. The seroprevalence of HBsAg as a marker of HBV infectiousness was 6.9% among anti-HBc positive individuals and 0.2% when extrapolated to the general Dutch population. Questionnaire data were used to estimate the anti-HBc prevalence in subgroups with certain known behavioural risk factors for HBV infection. A linear relationship was found between HBV infection and degree of urbanisation. Numbers were too small to show associations with several other known risk factors for HBV infection, except for ethnicity. Among foreign participants, especially the Turks, HBV infection was more common. Conclusion: This study is the first in the Netherlands to determine the prevalence of HBV infection in a representative sample of the Dutch population. For this reason, the anti-HBc and HBsAg seroprevalence are the most valid estimates for individuals in the Dutch population without obvious characteristics associated with higher risk for HBV infection. The prevalence of HBsAg is lower than previous estimates for the Dutch population, which were based on measurements in selected groups. The low HBsAg prevalence confirms the low endemicity level in the Netherlands.

Doel: Het bepalen van de seroprevalentie van hepatitis B virale (HBV) markers in de algemene Nederlandse bevolking. Methoden: 7395 sera uit de Pienter-serumbank, die tussen oktober 1995 en december 1996 is opgericht met een representatieve steekproef van de Nederlandse bevolking (17-79 jaar), zijn getest op aanwezigheid van anti-HBc en indien positief ook op HBsAg. Resultaten: De seroprevalentie van anti-HBc als een indicatie van huidige of doorgemaakte infectie was 2.1%. De seroprevalentie van HBsAg als een indicatie van HBV infectieusiteit was 6.9% bij anti-HBc positieve individuen en 0.2% wanneer dit werd gextrapoleerd naar de algemene Nederlandse bevolking. Vragenlijstgegevens zijn gebruikt om de anti-HBc prevalentie te schatten in subgroepen met bekende risicofactoren voor hepatitis B infectie. Een lineair verband werd gevonden tussen HBV infectie en urbanisatiegraad. De aantallen waren te klein om relaties aan te tonen met een aantal andere bekende risicofactoren voor HBV infectie, behalve voor allochtonen, in het bijzonder Turken, bij wie HBV infectie vaker voorkwam. Conclusie: Dit is de eerste Nederlandse studie waarbij de prevalentie van HBV infectie bepaald is in een representatieve steekproef van de Nederlandse bevolking. Daarom zijn de hier gevonden anti-HBc en HBsAg prevalentie de meest valide schattingen voor personen in de Nederlandse bevolking zonder duidelijke karakteristieken die geassocieerd zijn met een verhoogd risico op HBV infectie. De prevalentie van HBsAg is lager dan eerder schattingen die gebaseerd waren op metingen in geselecteerde groepen. De lage HBsAg prevalentie bevestigt het lage endemiciteitsnivo in Nederland
