Commissioned by the Dutch Health Care Inspectorate (IGZ), RIVM has developed an instrument for IGZ inspectors to explore to what extent Dutch hospitals have implemented and adopted a safety management system.
A safety management system provides hospitals with a means to gain insight into the risks patient may be subjected to during their stay in hospital. To reduce hospital stay-related risks, the minister of Health, Welfare and Sport initiated the hospital safety programme 'Prevent harm, work safely' some years ago. One of the requirements of the programme is that all Dutch hospitals must implement a safety management system.
This report presents the structure of inspection tool 'InViZie'. The tool comprises questionnaires for four different organisational levels in hospitals. Each question has a number of response options which can be scored. These scores provide the inspectors with a basis for their general judgement regarding the state of safety management systems in hospitals.
The instrument has not yet been tested as to its validity or reliability. Recommendations regarding the execution of such tests are presented. The first recommendation is to test the validity of the instrument. The second recommendation is analysing the association of better working safety management systems and patient safety.

Het RIVM heeft in opdracht van de Inspectie voor de Gezondheidszorg (IGZ) een instrument ontwikkeld voor inspecteurs van de IGZ. Met behulp van dit instrument kunnen zij erop toezien in hoeverre veiligheidsmanagementsystemen (VMS) in Nederlandse ziekenhuizen zijn geimplementeerd en goed worden uitgevoerd.
Het VMS verschaft ziekenhuizen inzicht in risico's die patienten kunnen lopen tijdens hun verblijf in een ziekenhuis. Om deze risico's zo veel mogelijk te beperken heeft de minister van Volksgezondheid, Welzijn en Sport enkele jaren geleden het veiligheidsprogramma 'Voorkom schade, werk veilig' geinitieerd. Daaruit vloeit voort dat alle Nederlandse ziekenhuizen een VMS dienen te hebben.
In dit rapport wordt de structuur van het inspectie-instrument InViZie beschreven. Dit instrument bestaat uit een aantal vragenlijsten die inspecteurs op vier verschillende organisatieniveaus in ziekenhuizen kunnen afnemen. Op basis van de antwoorden kan de inspecteur zich een algemeen oordeel vormen.
Het instrument is nog niet getoetst op validiteit of relevantie. In dit rapport worden aanbevelingen gedaan voor de uitvoering van deze toetsing. De eerste aanbeveling is dat de validiteit van het instrument wordt onderzocht. De tweede aanbeveling is te analyseren of een beter VMS samenhangt met betere patientveiligheid.
