Depression is twice as prevalent in diabetic patients as in nondiabetics. Improved recognition and treatment of depression in diabetic patients would help prevent diabetes-related complications. Current healthcare practices in the Netherlands are inadequate in terms of recognizing the problematic nature of this combination and therefore need revision. These are the conclusions of the National Institute for Public Health and the Environment (RIVM) based on a systematic literature review by order of the Dutch Ministry of Health, Welfare and Sport. The aim of this Ministry is to reduce the number of individuals with diabetes and diabetic complications.
In the Netherlands, one in every six individuals with diabetes presents with depressive symptoms yearly (approximately 100,000 people). Only 20-50% of these are recognized by healthcare providers as suffering from depression. Individuals with both depression and diabetes have an increased risk for diabetic complications, such as cardiovascular disease, kidney disease and eye disease. They also have a decreased quality of life, worse glycemic control and increased healthcare costs.
The treatment of depression in diabetic patients is doubly effective: not only may the depressive symptoms disappear, but treatment of the depression may also improve glycemic control. The RIVM therefore recommends that the Care Standard of the Dutch Diabetes Federation is modified with the aim of improving the awareness of healthcare providers to the symptoms of depression among the diabetic population. Current healthcare policy is aimed at self-management even though diabetics suffering from clinical depression show a reduced capacity to manage their diabetes.
This study also found that individuals with depression are at higher risk of developing diabetes, which provides support for implementing modifications in the standards for the treatment of depression as well.

Mensen met diabetes hebben twee keer zo vaak last van depressies als mensen zonder diabetes. Een betere herkenning en behandeling van depressies kan de complicaties van diabetes helpen voorkomen. Herziening van de diabeteszorg is nodig, omdat de huidige zorg deze gecombineerde problematiek te weinig onderkent. Dit concludeert het RIVM op basis van literatuuronderzoek in opdracht van het ministerie van Volksgezondheid, Welzijn en Sport (VWS). Dit ministerie wil het aantal mensen met diabetes en diabetesgerelateerde complicaties terugdringen.
Jaarlijks heeft een op de zes Nederlanders met diabetes last van depressieve symptomen; ruim honderdduizend mensen. Van hen wordt slechts twintig tot vijftig procent herkend binnen de diabeteszorg. Depressieve mensen met diabetes lopen een groter risico op complicaties van diabetes, zoals hart- en vaatziekten, nierziekten en oogziekten. Bovendien hebben zij een verminderde kwaliteit van leven, slechtere glucosewaarden en betalen zij meer zorgkosten.
De behandeling van een depressie bij mensen met diabetes blijkt dubbel effectief. Niet alleen de psychische klachten kunnen verdwijnen, er zijn ook aanwijzigen dat de glucosewaarden verbeteren. Het RIVM adviseert daarom de Zorgstandaard van de Nederlandse Diabetes Federatie aan te passen zodat er in de diabeteszorg aandacht is voor psychosociale problematiek. Het huidige beleid voor diabetes is ingezet op zelfzorg, hoewel mensen met diabetes en depressie minder goed in staat zijn om voor zichzelf te zorgen.
Uit het onderzoek blijkt bovendien dat mensen met een depressie meer kans op diabetes hebben. Dit pleit ervoor ook de richtlijnen voor depressiezorg aan te passen.
