The RIVM has developed a set of performance indicators for the chain of emergency care. The indicators can potentially reveal which part(s) of the chain present(s) the highest risk in terms of poor quality and a lack of safety. Performance indicators can provide an insight that will facilitate the Inspectorate's task of supervising the quality of care provided by health care organisations. The research was performed on request of the Dutch Health Care Inspectorate. Reason for the research was a number of shortcomings in the emergency care.
This report describes the development of this set of performance indicators. The set consists of 26 indicators. The indicators about trauma care were developed in cooperation with the VU University Medical Centre (Amsterdam). Experts from all levels of emergency care were actively involved in the development of the indicators. The indicators are focused on the accessibility, timeliness, safety, effectiveness and patient orientation of the emergency care provided for patients with an acute heart infarction, acute stroke and major trauma. Indicators were also developed for patients with an acute health problem in general.
To measure the performance indicators, important information is lacking. This is due to the lack of registration systems in which important data on the patient and the paramedical and medical procedures can be recorded in an efficient manner. A major improvement in the registration systems is therefore necessary. Professional and scientific associations are potential candidates for this task. Only when the registration systems are improved will it be possible to compile the data needed for calculating the indicators and thereby determine their quality.

Het RIVM heeft een samenhangende set prestatie-indicatoren ontwikkeld voor de keten van spoedeisende zorg. De indicatoren kunnen inzichtelijk maken waar in deze sector risico's bestaan op slechte kwaliteit en onveiligheid. De indicatoren zijn in opdracht van de Inspectie voor de Gezondheidszorg (IGZ) ontwikkeld en kunnen de IGZ ondersteunen bij haar toezichttaken. Aanleiding voor dit onderzoek waren geconstateerde tekortkomingen in de spoedeisende keten.
Dit rapport beschrijft hoe de set van prestatie-indicatoren tot stand is gekomen. De set bestaat uit 26 indicatoren, waarvan de indicatoren voor traumazorg in samenwerking met het VU Medisch Centrum zijn ontwikkeld. Bij de ontwikkeling van de indicatoren zijn deskundigen uit het hele veld van de spoedeisende keten betrokken. De indicatoren zijn gericht op de toegankelijkheid, tijdigheid, veiligheid, effectiviteit en patientgerichtheid van de spoedeisende zorg voor patienten met een acuut hartinfarct, hartstilstand, acute beroerte en ernstig trauma. Daarnaast zijn indicatoren ontwikkeld voor algemene spoedeisende gezondheidsproblemen.
Om de prestatie-indicatoren goed te kunnen meten ontbreekt evenwel belangrijke informatie. Dat komt omdat registratiesystemen ontbreken die belangrijke gegevens over de patient en de uitgevoerde verpleegkundige en medische handelingen vastleggen. Een sterke verbetering van de registratiesystemen is dan ook gewenst. Hier ligt een taak voor de wetenschappelijke verenigingen en brancheorganisaties. Pas als de informatievoorziening verbeterd is, kunnen de indicatorgegevens daadwerkelijk worden verzameld en kan de kwaliteit daarvan verder worden onderzocht.
