English Abstract
This report describes the development of a limited set of nine performance-indicators for diabetes type 2 care and the criteria used to do so. The associations between the disease history and the related health care processes are explored by means of a 'clinical logic'. A clinical logic involves two steps, first the distrubution of part of an initially healthy population over the main stages of the disease and second the determination of the effect of the health care process (including prevention) on the transmission rates between the disease stages.
In the present report the first step consisted of filling the clinical logic for diabetes with available population data. The second step has been interpreted as 'health expectancy that is theoretically achievable' and 'possibilities for affecting the transmission rate by prevention and care'. At present there is insufficient information about the actual performance of prevention and care as to diabetes. The framework of the clinical logic is mainly used to come to a selection of performance-indicators of diabetes prevention and care. This set of indicators will enable the Ministery of Health to keep a finger on the pulse of diabetes care.

Rapport in het kort
In dit rapport is een beperkte set van negen prestatie-indicatoren voor diabetes type 2 ontwikkeld. Het rapport beschrijft de systematiek om tot de set van prestatie-indicatoren te komen. De samenhang is in kaart gebracht tussen ziektegeschiedenissen en zorgprocessen in de vorm van een 'clinical logic'. Hierin wordt nagegaan hoe een deel van de aanvankelijk gezonde populatie zich verdeelt over verschillende belangrijke (voor)stadia van diabetes en wat de invloed is van het zorgproces (inclusief preventie) op de 'overgangskansen' tussen de ziektestadia.
In dit rapport is een begin gemaakt met het invullen van de clinical logic voor diabetes door het te vullen met beschikbare populatiegegevens. De tweede stap (invloed van het zorgproces) is in dit rapport opgevat als ' in theorie nog te behalen gezondheidswinst' en 'mogelijkheden voor beonvloeding door preventie en zorg'. Er is op dit moment namelijk nog onvoldoende informatie beschikbaar over de feitelijke prestaties van preventie en zorg bij diabetes. Het kader van de clinical logic is vooral gebruikt om tot een selectie van indicatoren voor prestaties van preventie en zorg bij diabetes te komen. Met deze set van prestatie-indicatoren kan het ministerie van VWS een vinger aan de pols houden bij de zorg voor diabetes.
