English Abstract One of the goals of the Dutch government is to reduce
the consumption of tobacco products and protect the non-smoker, for example,
by creating smoke-free workplaces in the hospitality industry. Currently,
dilution ventilation is the usual application in the hospitality industry.
However, with the use of optimal ventilation (replacement ventilation rather
than dilution ventilation, with correct air exchange rates and separation
between smokers and non-smokers areas) and air cleaning, quite considerable
reductions of exposure to environmental tobacco smoke can be achieved. The
hospitality industry is however, far from achieving this situation.
Improving the use of existing dilution equipment up to the level of the
Dutch construction requirements can lead to a roughly estimated reduction of
exposure to environmental tobacco smoke of several tens of percents. Lack
of proper use of sophisticated ventilation and air cleaning technologies
will be a serious threat to achieving possible reductions in exposure. In
all cases however, exposure to environmental tobacco smoke will remain. A
'safe' level of exposure was not found in the scientific literature. As
dose-response curves are not known for environmental tobacco smoke, it is
not possible to quantify the reduction in the health risk by reducing the
exposure of environmental tobacco smoke through ventilation or air
cleaning.

Naar aanleiding van het beleid van de rijksoverheid om
het tabaksgebruik te verminderen en de niet-roker te beschermen
(bijvoorbeeld door het creeren van rookvrije werkplekken) is de vraag
ontstaan in welke mate ventilatie en luchtzuivering kunnen bijdragen aan
vermindering van de blootstelling aan omgevingstabaksrook in de horeca. In
de horeca worden ventilatie technieken gebruikt die zijn gebaseerd op
menging en verdunning (raam- en muurventilatoren bijvoorbeeld). Met
optimale ventilatie (verdringing met de juiste volumestromen in plaats van
mengventilatie en fysieke scheiding) en luchtreiniging is in principe een
aanzienlijke reductie van de blootstelling aan omgevingstabaksrook te
bewerkstelligen. De huidige praktijk in de horeca is daar erg ver van
verwijderd. Met vigerende ventilatietechnieken in de horeca zijn reducties
van de blootstelling tot maximaal enkele tientallen procenten te bereiken
als optimaal gebruik gemaakt gaat worden van de ventilatie-eisen die in het
Bouwbesluit zijn beschreven. Inadequaat gebruik van geavanceerd ventilatie-
en luchtreinigingstechnieken vormt een serieuze bedreiging voor de optimaal
te bereiken reductie van de blootstelling aan omgevingstabaksrook. Een
veilig niveau van blootstelling aan omgevingstabaksrook is niet aan te geven
op basis van beschikbare literatuur. Volledige reductie van het
gezondheidsrisico door ventilatie zal dan ook niet mogelijk zijn. Aangezien
precieze blootstelling-effect-relaties niet bekend zijn voor
omgevingstabaksrook, is niet te kwantificeren in welke mate reductie van het
gezondheidsrisico daadwerkelijk bereikt kan worden door reductie van de
blootstelling.
