Efficiency en uitkomst van onaangekondigde GMP-inspecties bij farmaceutische bedrijven

The findings of unannounced inspections of pharmaceutical manufacturers do not differ from those that have been announced beforehand. The reason for this is most likely due to the quality control system of these companies being very strictly organized. This was the conclusion of a study carried out by the RIVM, by order of the Dutch Health Care Inspectorate (Inspectie voor de Gezondheidszorg, IGZ). The findings are based on random spot checks of 60 pharmaceutical manufacturers.

The advantages of unannounced inspections in terms of the inspection process itself are that less time is required to plan the inspections and inspectors can manage their time in a more flexible manner. Consequently, unannounced inspections of pharmaceutical manufacturers can be viewed as complementary to the inspection methods of the IGZ. A disadvantage of unannounced inspections is that there is always the possibility that management/employees germane to the inspection and/or manufacturing processes are absent at the time of the inspection or that the manufacturing processes for inspection have been stopped or are not in process at that time.

In this study, the RIVM also surveyed the experiences of the inspectors as well as the inspected manufacturers with respect to unannounced inspections. The inspectors have a positive opinion of unannounced inspections as an inspection tool and have no experience of the findings from unannounced and announced inspections being different. However, the inspectors do consider that, dependent on the risk associated with the inspection, they should be able to decide for themselves whether the inspection should be announced or unannounced. The viewpoint of pharmaceutical manufacturers is more neutral: while they have no problem with unannounced inspections, this is clearly not their preferred inspection procedure. Each pharmaceutical manufacturer in the Netherlands is inspected once in 3 years by the IGZ to determine whether the manufacturer complies with the applicable legal and regulatory requirements relating to the quality control system (the so-called GMP-Inspection). There are approximately 300 such companies in the Netherlands for which such GMP-Inspections need to be carried out. The request by the IGZ for this study is in line with the recent development to provide a scientific basis to inspection supervision methods.

Als farmaceutische bedrijven onaangekondigd worden geïnspecteerd verschillen de bevindingen niet van inspecties die vooraf worden aangekondigd. Dit komt waarschijnlijk doordat het systeem voor kwaliteitscontrole voor deze bedrijven heel strikt is georganiseerd. Dit blijkt uit onderzoek van het RIVM, in opdracht van de Inspectie voor de Gezondheidszorg (IGZ). De bevindingen zijn gebaseerd op een steekproef onder zestig farmaceutische bedrijven.

Onaangekondigde inspecties hebben voor de inspectie als voordeel dat het minder tijd kost om ze te plannen en inspecteurs flexibeler met hun tijd kunnen omgaan. Daarom kunnen onaangekondigde inspecties van farmaceutische bedrijven als een aanvulling op de inspectiemethoden van de IGZ worden beschouwd. Een nadeel van onaangekondigde inspecties is dat een inspecteur voor niets kan komen als relevante medewerkers afwezig zijn of activiteiten bij het bedrijf zijn gestaakt.

Ook is onderzocht hoe zowel de inspecteurs als de geïnspecteerde bedrijven de onaangekondigde inspecties beleven. De inspecteurs ervaren geen wezenlijke verschillen bij onaangekondigd inspecteren. Ze zijn positief over onaangekondigde inspecties als inspectie-instrument. Wel willen ze, afhankelijk van de verwachte risico's, zelf kunnen bepalen of de inspectie aangekondigd dan wel onaangekondigd zal plaatsvinden. De farmaceutische bedrijven staan neutraal tegenover onaangekondigde inspecties: ze vinden het geen probleem, maar het heeft ook niet duidelijk hun voorkeur.

De IGZ controleert één keer in de drie jaar of een farmaceutisch bedrijf de wettelijke eisen naleeft die hun kwaliteitssysteem vereist (zogeheten GMP-inspecties). In Nederland gaat het in totaal om circa driehonderd van deze bedrijven. Het verzoek van de IGZ om het onderzoek uit te voeren past in de recente ontwikkeling om methoden van inspectietoezicht wetenschappelijk te onderbouwen.
