English Abstract For the Public Health Status and Forecasts 1997
attention has been given to the health benefit that could still be achieved
by primary and secondary prevention. For selected examples of five types of
methods of prevention (health education, vaccination, legislation and
regulation, pharmaceutical prophylaxis and screening) the efficacy (the
health benefit which can be achieved under optimal circumstances) is
compared to the effectiveness (the health benefit which is achieved in
practice). The potential health benefit through interventions aimed at
(life-style related) determinants is considerable. It would be advisable to
work towards a programmatic prevention strategy giving attention to
ascertaining the correct target group and the most suitable intervention
strategy, to regular evaluation of the effectiveness, to implementing an
intersectoral approach and to clearly establishing the responsibilities at
both the central and local level.

Dit rapport is gewijd aan de vragen: Hoeveel
gezondheidswinst is nog te behalen met preventie? Welke interventies
leveren de meeste winst? Deze vragen worden opgeroepen nu steeds meer
blijkt dat financiele beperkingen dwingen tot het maken van keuzen. Voor
een vijftal interventiemethoden (GVO, vaccinatie, wet- en regelgeving,
primaire preventie door gerichte medicatie en screening) is aan de hand van
illustratieve voorbeelden gekeken naar de theoretisch te behalen en
feitelijk behaalde gezondheidswinst en naar de knelpunten die zich voordoen
bij het in de praktijk realiseren van gezondheidswinst. Met preventie is in
het verleden al veel gezondheidswinst behaald. Om deze winst te behouden en
verder uit te breiden is een programmatische preventiestrategie nodig,
waarin door middel van een strakke regie en participatie van alle betrokken
partijen programma's worden opgezet die optimaal zijn afgestemd op de
doelgroep en die door regelmatige evaluatie worden getoetst op
doeltreffendheid.
