English Abstract Public health nurses at the Community Health Services
maintain a register of the palliative treatment of sexually transmitted
diseases (STD). Fifty Community Health Services registrered 9,888
consultations in 1997 (15% less than in 1996). Forty-six Community Health
Services registered 10,123 consultations in 1998 (2% more than in 1997).
Half of these consultations was for an STD examination, and the other half
for a HIV test or both. About three-quarters of the patients were of Dutch
descent. In 1997, three-quarters of the male patients had had heterosexual
contacts, and 11% had had homosexual contacts; 11% were prostitutes.
Twenty percent of the women worked as prostitutes in both 1997 and 1998.
Most of the consultations take place because of symptoms, risk behaviour, or
the beginning of a new relationship. Most of the patients were between 20
and 34 years old. The number of cases of STD diagnosed decreased in 1997
with respect to 1996 by 25% to 2213. In 1998, 2844 cases of STD were
diagnosed (an increase of 29% with respect to 1997). It was striking that
those in the group for which STD was confirmed were less often of Dutch
descent, had homosexual contacts more often, were more often prostitutes, or
worked more often in the prostitiution world than those in the group for
whom STD was not found. The most common STD dor both men and women is
chlamydia; the next most common STD for men was genital warts and
gonorrhoea. The next most common STD for women were candidiasis, bacterial
vaginitis and genital warts. There were requests for HIV tests in 4667
consultations (precounseling) in 1997, of which 0.7% were positive, and
0.8% of the 4667 HIV tests were positive in 1998.

Bij GGD's wordt door sociaal-verpleegkundigen een
registraite bijgehouden van de bezoekers ten behoeve van een SOA-hulpvraag
of HIV-testverzoek. In 1997 registreerden 50 GGD's 9.888 consulten (daling
van 13% t.o.v. 1996). In 1998 registreerden 46 GGD's 10.123 consulten
(stijging van 2% t.o.v. 1997). Van deze consulten was de helft t.b.v. een
SOA onderzoek, de andere helft t.b.v. een HIV test of voor beide. Ruim
driekwart van de bezoekers is oorspronkelijk afkomstig uit Nederland. In
1997 had driekwart van de mannelijke bezoekers hetroseksueel en 11%
homoseksueel contact gehad, 11% was prostitiuant. In 1998 had 14%
homoseksueel cotnact gehad. Van de vrouwen werkte zowel in 1997 als in 1998
20% als prostituee. De meeste consulten voor SOA vinden plaats vanwege
klachten, eigen risicogedrag en aanvang van een nieuwe relatie. De meeste
bezoekers zijn tussen de 20 en 34 jaar. Het aantal gediagnosticeerde SOA
daalde in 1997 met 25% tot 2213. In 1998 werden 2844 SOA gediagnosticeerd
(stijging van 29% t.o.v. 1997). Opallend is dat de groep waarbij een SOA
is vastgesteld minder vaak uit Nederland afkomstig is, vaker homoseksuele
contacten heeft, vaker prostituant was en vaker in de prostitutie werkzaam
was dan de groep bezoekers waarbij geen SOA is vastgesteld. De meest
voorkomende SOA bij zowel mannen als vrouwen is chlamydia; bij mannen
gevolgd door genitale wratten en gonorroe en bij vrouwen gevolgd door
candidiasis, bacteriele vaginose en genitale wratten. In 1997 was bij 4667
consulten sprak van een HIV-testverzoek (precounseling) waarbij 0,7%
positief bleek te zijn, in 1998 was 0,8% van de 4667 HIV-testen
positief.
