English Abstract Member States of the European Union are required to
determine the exposure of the population to ionising radiation used in
medicine. This requirement, recently been implemented in Dutch legislation,
is laid down in an EU Directive on health protection of individuals against
the dangers of ionising radiation in relation to medical exposure. To
fulfil this requirement, RIVM is developing an information system on medical
radiation applications (IMS). Here, the data collection on three issues
relevant for the IMS will be presented as a follow-up to a pilot study
previously conducted. The issues focus on the number of procedures in
hospitals, the age and sex distribution of the exposed patients, and the
relationship between staffing in the hospital departments and the number of
procedures. The three data sources used in this report are the annual
hospital survey (EJZ), data from health insurance companies and the annual
survey on staffing in hospitals.The total annual number of medical
procedures with ionising radiation in the IMS is (partly) based on
information from the EJZ. At the end of the nineties, the annual number of
diagnostic x-ray and nuclear medicine procedures was reasonably stable at
8.5 million and 240,000, respectively. The number of CT examinations at
that time showed an annual increase of about 1.8 % per year, which was, for
example, 494,000 in 1998.Data on health insurance claims, obtained for
about 4 million people for the year 2000, provide insight in the
distribution of different procedures and also into the age and sex
distribution of the patients who undergo these procedures. The age
distribution of patients undergoing CT examinations indicates that the
increase in these examinations at the end of the nineties corresponded to
the increase expected because of the growth and ageing of the population.
The number of x-ray and nuclear medicine examinations did not keep up with
these demographic developments.The data from the survey on staffing in
hospitals in 1997 was used to assess the relationship between the size of
the medical staff and the number of medical procedures. Results showed
differences between hospitals to be considerable, although the data are not
comprehensive enough to draw conclusions from this.

Lidstaten van de Europese Gemeenschap moeten inzicht
hebben in de blootstelling van hun bevolking aan ioniserende straling als
gevolg van medische handelingen. Deze regel is vastgelegd in de Europese
richtlijn ter bescherming van patienten, die recent in de Nederlandse
wetgeving is geimplementeerd. Voor dit doel ontwikkelt RIVM een
Informatiesysteem Medische Stralingstoepassingen (IMS). In dit rapport
worden voor drie onderwerpen gegevens voor het IMS gepresenteerd als vervolg
op een eerdere pilotstudie. Deze drie onderwerpen zijn aantallen
verrichtingen, leeftijds- en geslachtopbouw van blootgestelde patienten en
de relatie personele bezetting en aantallen verrichtingen. De drie
informatiebronnen waar we voor dit rapport uit geput hebben zijn de Enquete
Jaarcijfers Ziekenhuizen (EJZ), declaratiegegevens van
ziektekostenverzekeraars en de te Personeelssterkte. De gegevens uit de EJZ
tonen aan dat het totale aantal rontgen-, en nucleair geneeskundige
onderzoeken eind jaren negentig vrij stabiel blijft, respectievelijk 8,5
miljoen en 240 duizend onderzoeken per jaar. Het aantal CT onderzoeken
neemt licht toe, ongeveer 1,8% per jaar, tot 494 duizend in 1998. Van
ziektekostenverzekeraars zijn declaratiegegevens over 2000 verkregen van
ongeveer vier miljoen verzekerden. Deze gegevens geven inzicht in het
verband tussen verschillende verrichtingen en de leeftijds-, en
geslachtsverdeling van betrokken patienten. De toename van het aantal CT
onderzoeken in de tweede helft van de jaren negentig loopt gelijk op met de
toename die te verwachten was op grond van bevolkingsgroei en vergrijzing.
De aantallen rontgen- en nucleair geneeskundige onderzoeken zijn daarentegen
achtergebleven bij de verwachte toename.Met gegevens uit de Enquete
Personeelssterkte is gekeken in hoeverre de personele bezetting en het
aantal verrichtingen aan elkaar gerelateerd zijn. De relatie tussen het
aantal verrichtingen en de personeelssterkte in 1997 laat grote verschillen
zien tussen ziekenhuizen, maar de gegevens zijn niet gedetailleerd genoeg om
daar conclusies aan te verbinden.
