Samenvatting
Het doel van het hepatitis B-vaccinatieprogramma (HBV-programma) is te voorkomen dat hepatitis B wordt overgedragen. Deze gratis vaccinatie is bedoeld voor mannen die seks hebben met mannen (MSM) en sekswerkers, omdat zij een grotere kans hebben om hepatitis B te krijgen. Vaccinatie voorkomt nieuwe infecties. Ook wordt met het programma uitgezocht welke mensen hepatitis B hebben, zodat zij medische zorg kunnen krijgen en het virus niet meer kunnen verspreiden. Het RIVM geeft een overzicht van wat er in de jaren 2022, 2023 en 2024 voor het HBV (hepatitis B virus)-programma is gebeurd.
In 2022 waren veel GGD (Gemeentelijke Gezondheidsdienst)'en nog druk met de nasleep van de coronapandemie, waardoor er minder tijd was om de doelgroepen voor de hepatitis B-vaccinatie te bereiken. In 2024 is dat hersteld en gaf de GGD evenveel vaccinaties als voor corona. Sekswerkers worden steeds moeilijker te bereiken, omdat zij minder zichtbaar zijn. Dat komt bijvoorbeeld doordat sekswerkers vaker van werkplek veranderen of vanuit huis werken. Ook maken iets minder mensen, vooral sekswerkers, hun serie van drie prikken af. Dan werkt de vaccinatie iets minder goed. Verder is het registratiesysteem aangepast om andere genders dan man en vrouw te kunnen registreren.
Soa Aids Nederland begon in 2023 een online campagne 'Hep je B?' om de doelgroepen ook op deze manier te informeren over de hepatitis Bvaccinatie. Een analyse laat zien dat het goed werkt om risicogroepen via online communicatie te informeren. Wel blijft het belangrijk dat GGD'en kijken naar wat er in hun regio werkt. GGD Zuid-Limburg zette zich in om meer MSM (mannen die seks hebben met mannen) met een migratieachtergrond te bereiken.
In 2024 is het draaiboek van het HBV-programma geüpdatet. Ook zijn online inspiratiesessies gehouden waarin GGD'en ervaringen konden delen. Soa Aids Nederland startte de nieuwe campagne 'Zeg nee tegen hepatitis B', om jonge MSM te bereiken.
In 2011 is de hepatitis B-vaccinatie toegevoegd in het Rijksvaccinatieprogramma (RVP). Daardoor is de kans op hepatitis B voor jongere generaties veel kleiner. Voor de toekomst blijft het belangrijk om mensen te bereiken die nog niet zijn gevaccineerd.
Abstract
The objective of the hepatitis B vaccination programme (HBV programme) is to prevent hepatitis B transmission. This free vaccination is for men who have sex with men (MSM) and sex workers, as they are at elevated risk of contracting hepatitis B. Vaccination prevents new infections. The programme is also intended to identify people with hepatitis B so that they can receive medical care and are no longer at risk of spreading the virus. Below, RIVM provides an overview of all steps taken for the HBV programme in 2022, 2023 and 2024.
In 2022, many Municipal Public Health Services (PHS) were still dealing with the aftermath of the COVID-19 pandemic, leaving less time to reach the target groups for the hepatitis B vaccination. This was remedied in 2024 when the PHS administered the same number of vaccinations as before the pandemic. Vaccinating sex workers has become increasingly difficult, as they have become less visible. Reasons include the fact that sex workers often change their work location or work from home. Also, a slightly lower number of people - sex workers in particular - do not get all three doses of the vaccine. This affects the efficacy of the vaccination. Furthermore, the registration system was adapted to register genders other than male and female.
In 2023, Soa Aids Nederland launched an online campaign entitled Hep je B? (Do you have hepatitis B?) to inform the target groups about the hepatitis B vaccination. An analysis shows that informing risk groups through online communications is effective. That said, PHS do need to keep an eye on effective ways of communicating in their regions. PHS Zuid-Limburg made an effort to reach more MSM with a migration background.
The guideline of the HBV programme was updated in 2024. Online inspiration sessions were organised for PHS to share their experiences. Soa Aids Nederland launched a new campaign entitled Zeg nee tegen hepatitis B (Say no to hepatitis B) in order to reach young MSM.
In 2011, the hepatitis B vaccination was added to the National Immunisation Programme (NIP), greatly reducing the risk of contracting Hepatitis B for younger generations. Going forward, it remains key to reach as yet unvaccinated people.
