Samenvatting
De pneumokokkenbacterie veroorzaakt ernstige infecties zoals longontsteking, bloedvergiftiging en hersenvliesontsteking. Ernstige pneumokokkenziekte komt in Nederland elk jaar voor bij naar schatting zo’n 1600 mensen van 60 jaar en ouder. Om de ziekte te voorkomen kunnen mensen van deze leeftijd zich laten vaccineren in het Nationaal Programma Pneumokokken Vaccinatie (NPPV). Kinderen worden tegen pneumokokken gevaccineerd via het Rijksvaccinatieprogramma (RVP). De vaccins voor kinderen en ouderen verschillen en werken voor een deel tegen dezelfde typen pneumokokken.
Er zijn veel verschillende pneumokokken. De vaccines richten zich op de typen pneumokokken waar mensen heel ziek van kunnen worden. Pneumokokken worden vooral door kinderen verspreid. Door hen te vaccineren, verdwijnen de typen pneumokokken waartegen kinderen worden gevaccineerd en kunnen deze zich minder verspreiden. Op deze manier worden ouderen indirect ook tegen deze typen beschermd. Wel kunnen de typen pneumokokken die niet in het kindervaccin zitten, na de vaccinaties vaker voorkomen (typeverandering). Van deze typen kunnen mensen soms ook heel ziek worden.
Sinds de herfst van 2025 krijgen 60-plussers een vaccin dat gericht is op 20 typen pneumokokken (PCV20). Daarvan zijn 15 typen hetzelfde als in het vaccin dat bij kinderen wordt gebruikt (PCV15), plus vijf andere. Sinds kort bestaat er een vaccin tegen 21 pneumokoktypen (PCV21). Dit vaccin overlapt veel minder met het PCV15-vaccin voor kinderen. Het beschermt daarom volwassenen ook tegen de typen pneumokokken die als gevolg van kindervaccinatie meer zouden kunnen gaan circuleren.
Dit en meer staat in het achtergronddocument dat het RIVM voor de Gezondheidsraad heeft verzameld. Dit document dient als ondersteuning voor het advies van de Gezondheidsraad over welk vaccin het beste is voor het NPPV. Het ministerie van (Ministerie van Volksgezondheid, Welzijn en Sport) heeft de Gezondheidsraad gevraagd hierover te adviseren.
Het RIVM beschrijft bijvoorbeeld hoe vaak ernstige pneumokokkenziekte voorkomt bij 60-plussers, hoe veilig PCV21 is en hoe goed het werkt. Daarnaast staat beschreven welke chronische ziektes ervoor zorgen dat mensen sneller pneumokokkenziekte krijgen.
Abstract
The bacterium pneumococcus can cause serious infections such as pneumonia, sepsis and meningitis. In the Netherlands, about 1,600 people aged 60 years or older get a serious pneumococcal infection every year. To prevent pneumococcal disease, people in this age group can be vaccinated through the National Pneumococcal Vaccination Programme (NPPV). Children are offered vaccination against pneumococcal disease as part of the National Immunisation Programme (NIP). The vaccines for children and older adults are different, but also protect against some of the same types of pneumococci.
While many types of pneumococci exist, the vaccines focus on those with the highest risk of causing severe illness. Pneumococci are spread primarily by children. Childhood vaccination reduces the prevalence and transmission of the types included in the NIP-vaccine, thereby also indirectly protecting older adults against disease caused by these types. However, types of pneumococci not included in the childhood vaccine may become more common after vaccination and can also cause severe illness. This is known as ‘serotype replacement’.
Since autumn of 2025, people aged 60 years or older have been offered a PCV20 vaccine targeting 20 types of pneumococci. Fifteen of these types are identical to those included in the PCV15 vaccine used for children; the other five are not. A new vaccine that protects against 21 types of pneumococci, PCV21, has recently become available. Because PCV21 has less overlap with the PCV15 vaccine used for children, it also protects adults against types of pneumococci that may spread more as a result of childhood vaccination.
This information and more is included in a background document prepared by the National Institute for Public Health and the Environment (RIVM) for the Health Council of the Netherlands. The document supports the Health Council’s advice on which vaccine is best suited to the NPPV. The Ministry of Health, Welfare and Sport (VWS) asked the Health Council to advise on this matter.
The RIVM document explains how often serious pneumococcal disease occurs in older adults, how safe PCV21 is and how effective it is. It also describes the chronic diseases that increase people’s risk of pneumococcal disease.
