* The percentage of positive tests is no longer clear and unambiguous to interpret and compare over time. This is partly due to testing for confirmation after a positive self-test, and testing of returning travellers.
** A self-test is recommended for people without symptoms, for the purpose of periodic (preventive) testing when required for school or work and upon returning from holiday. Since 3 December, a self-test can also be used in the event of mild symptoms and prior to a visit. People who test positive using a self-test are advised to get tested by the Municipal Public Health Services (GGDs) to confirm the positive test result and exclude the possibility of a false positive on the self-test. It is not unlikely that not everyone will go to the GGD for a confirmation test after a positive self-test. More information on the use and results of self-tests can be found at Government.nl.
*** This is the R number for 4 December 2021. NICE data updates are provided twice a week (on Wednesdays and Saturdays). Therefore, these R numbers were calculated based on the NICE data with the data available on Saturday 18 December.
¹GGD reports to RIVMNational Institute for Public Health and the Environment between 14 December 10:01 and 21 December 10:00 as published 21 December 2021 in the weekly epidemiological update on SARS-CoV-2 in the Netherlands.

3As published 21 December 2021 in the weekly epidemiological update on SARS-CoV-2 in the Netherlands. The figures may be updated to include any registrations provided later by the NICE Foundation, especially regarding the past week and the week before that.
4Not all of these reported deaths of COVID-19 patients occurred within the last week. The actual number of deaths is higher than the deaths that are reported in the surveillance. This is because there is no mandatory reporting requirement for death due to COVID-19. For that reason, the mortality figures presented here are underreported.

OMT advisory reports - 19 November 2021
On 19 November 2021, the Outbreak Management Team (OMT) convened to consult on the COVID-19 outbreak. Among other topics, the OMT discussed the following:
- The course of the epidemic
- Reproduction number R and forecasts for hospital and ICU admissions
- Measures
- Effectiveness of the coronavirus entry pass
- The role of self-testing in the testing landscape
- Closing time for cafés and restaurants
The OMT urgently emphasises that following the basic rules is the only way to prevent additional measures from becoming necessary this winter. The first priority is staying home and getting tested if you have symptoms, supplemented by other measures, such as staying 1.5 metres apart and wearing face masks that cover the mouth and nose in locations where distancing is not possible. Also, the OMT indicates that the use of the coronavirus entry pass is a mitigation tool intended to limit risks when opening sectors, not a preventative measure to keep the virus from spreading.

Trend continues: 7 in 10 COVID-19 patients in ICU are not vaccinated
New analyses of hospital and ICU admissions show that COVID-19 vaccination continues to be very effective. In the period from 11 July to 14 November, vaccine effectiveness was 94% against hospital admission and 97% against ICU admission. Vaccine effectiveness measured over the past eight weeks was 92% against hospital admission and 96% against ICU admission.
In the past eight weeks, the chance that a fully vaccinated person with the coronavirus SARS-CoV-2 will be admitted to hospital was 12.5 times lower than for a non-vaccinated person. The chance of ICU admission for a fully vaccinated person was 25 times lower. These results show that there are no major changes in vaccine effectiveness compared to previous analyses in early November 2021.
There are several possible explanations for why vaccine effectiveness has been slightly lower in the last 8 weeks compared to the entire period from 11 July to 14 November 2021. One possibility is that people who have recently been admitted to hospital were, on average, vaccinated longer ago than people who were admitted at an earlier point in the year. The higher infection rate in recent weeks may also explain it.
Vaccine effectiveness against hospital admission in the age group of 70 years and older was 86% in the past eight weeks.
The new data also shows that the incidence of both hospital and ICU admissions in the age group of 50-69 years primarily increased among unvaccinated people (Figure 1). In the age group of 70 and older, the incidence of admissions also increased among fully vaccinated people, although it is still many times higher among unvaccinated people.
Figure 1. Incidence per 100,000 people of the number of COVID-19 (A) hospital admissions and (B) ICU admissions among unvaccinated, partly vaccinated and fully vaccinated people, by age group. Due to reporting delays, the data for the past few days is not yet complete.
Data used for this page comes from NICE data on hospital admissions of patients with COVID-19 admitted until 14 November 2021, enriched on 15 November 2021 with vaccination data from the central COVID-19 vaccination register (CIMS). Further details on how vaccine effectiveness is calculated can be found in the report: Effectiveness of COVID-19 vaccination against hospital and ICU admission in the Netherlands (admissions 11 July - 14 November 2021) (report in Dutch). Please note: underlying data cannot be published due to its privacy-sensitive nature.

Total mortality significantly elevated
In the week from 28 October to 3 November 2021, total mortality in the Netherlands increased sharply (mortality reported within 2 weeks: around 97% deaths reported). A total of 3,365 deaths were reported in that week, while we would normally expect between 2,651 and 2,977 deaths per week at this time of year. * The percentage of positive tests is no longer clear and unambiguous to interpret and compare over time. This is partly due to testing for confirmation after a positive self-test, and testing of returning travellers.
** This is the R number for 23 October 2021. NICE data updates are provided twice a week (on Wednesdays and Saturdays). Therefore, these R numbers were calculated based on the NICE data with the data available on Saturday 30 October.

² GGD reports to RIVM between 26 October 10:01 and 2 November 10:00 as published 2 November 2021 in the weekly epidemiological update on SARS-CoV-2 in the Netherlands.
³ As published 9 November 2021 in the weekly epidemiological update on SARS-CoV-2 in the Netherlands. The figures from the past week and current week may be updated to include any registrations provided later by the NICE Foundation.
4 Not all of these reported deaths of COVID-19 patients occurred within the last week. The actual number of deaths is higher than the deaths that are reported in the surveillance. This is because there is no mandatory reporting requirement for death due to COVID-19. For that reason, the mortality figures presented here are underreported.
5 As published 9 November 2021 in the weekly epidemiological update on SARS-CoV-2 in the Netherlands. The figures from the past week and current week may be updated to include any test results provided later. These figures only include tests of people who were tested in the GGD test lanes and does not include any tests in the context of testing for access or in order to travel.
6 “Other tests” includes people who are known not to have been tested after taking a self-test, and people for whom it is not known whether they were tested after taking a self-test.

Graph of different variants no longer updated
The national pathogen surveillance shows that the relative prevalence of the different variants has been very stable for some time now; over 99.9% of the samples now involve the Delta variant. As a result, the graph tracking the different variants no longer displays any changes, and will not be updated for the time being. Pathogen surveillance will obviously continue and the table with the results of analysed samples and the downloadable pdf are updated every week. The graph will be updated again to reflect any future changes in variant prevalence.

***Last week was the last time that the number of contagious people was reported for the time being, since this figure is based on the number of hospital admissions. In this phase of the epidemic, when more and more people are vaccinated, and therefore fewer people are admitted to hospital, the number of infections in the general population is increasingly out of sync with the number of hospital admissions. That is why the number of contagious people is no longer calculated.

Pharmacovigilance Centre Lareb: biweekly report
In its biweekly report, Pharmacovigilance Centre Lareb stated that the centre had received a total of 93,453 reports by 4 July 2021, involving 498,983 possible side-effects. Most of the reported symptoms are in line with side-effects that were already known, such as headache, muscle pain and reactions at the injection site. Since the start of the vaccination campaign, Lareb has received a total of 448 reports regarding deaths shortly after receiving the COVID-19 vaccinations. Most of the reports involved the Pfizer/BioNTech vaccine. It is the COVID-19 vaccine that has been used most frequently, and is also the vaccine that is mainly given to the elderly. A news item on the Lareb website announced there have now been 34 reports of blood clots in combination with low platelets after vaccination (32 after AstraZeneca, 2 after Janssen). In 18 of these reported cases, it is almost certain that the rare side effect known as Thrombosis with Thrombocytopenia Syndrome (TTS) was involved.

Lareb: reported deaths following COVID-19 vaccination
Pharmacovigilance Centre Lareb published an overview today regarding the reported deaths in the first 14 weeks of the vaccination campaign (up to 15 April 2021). The overview does not suggest any pattern of symptoms that indicate severe side-effects leading to death, other than the previously identified rare side-effect of thrombosis with thrombocytopenia syndrome (TTS). In some cases, side-effects may have contributed to exacerbation of an already vulnerable health situation.

Researching the risk of COVID-19 during air travel
RIVM and the Netherlands Aerospace Centre (Royal NLR) conducted a study on the risk that a person will become ill by breathing in the coronavirus SARS-CoV-2 while on board an aircraft. The current measures, such as staying home if you have symptoms and showing a negative test and/or health statement before boarding, reduce the risk that someone who has the virus will board a plane. If someone who has the virus does board, the risk of contracting COVID-19 is relatively low for passengers in the seven rows around the contagious passenger. The risk is lower than in an indoor space with similar dimensions that is not ventilated, for example. The risk decreases proportionate to the distance from the contagious passenger. This study did not look at transmission of the virus via direct contact and surfaces.
RIVM estimated the risk based on measurements and computer simulations provided by the NLR. Various scenarios were run using mannequins in three types of aircraft cabins with seven rows. A ‘contagious passenger’ was seated in the middle row, exhaling artificial saliva that did not contain the actual virus. Measurements in the cabins showed how many droplets reached the mannequins.
Do you have symptoms? Stay home and get tested
Measures to reduce the risk that an infected passenger might board an aircraft are still important. For travellers: Do you have symptoms? Stay home and get tested. If you test positive, go into home isolation.

Model study on effectiveness of the CoronaMelder app
In its first few months, the CoronaMelder app made a positive but minor contribution to controlling the coronavirus SARS-CoV-2. This is the conclusion of an RIVM evaluation of the effect of CoronaMelder in combination with testing and standard source and contact tracing.
The effect of the app between 1 December 2020 and 31 March 2021 was estimated using modelling. From 1 December 2020 to 31 March 2021, 7514 people tested positive for COVID-19 after a notification from the CoronaMelder app. It is estimated that these notifications prevented over 15,000 infections. The R number (the average number of new infections caused by an infected person) was 12.7% lower due to the combination of testing, standard source and contact tracing and the CoronaMelder app. The app contributed 0.3% to this percentage.

5 As published 13 April 2021 in the weekly epidemiological update on SARS-CoV-2 in the Netherlands. The figures from the past week and current week may be updated to include any test results provided later by the NICE Foundation.
The number of newly reported infections that RIVM received last week from the Municipal Public Health Services (GGDs) is not the same as the number of positive tests in that week. The number of positive tests during the week could still change in response to delayed reports. These are newly reported infections that originate from test lanes, but also from hospitals and other sources. The number of infections in a week is expected to be higher than the number of positive tests, because it is likely that not everyone who is infected will get tested.
Not everyone who tests positive for COVID-19 is referred to as a patient. That term is used for people who are so ill that they are being monitored by a doctor, or for people who became so ill that they died. Not everyone who tests positive for COVID-19 becomes that ill. Many people only develop mild symptoms, or do not notice any symptoms at all. In the past, people who tested positive for COVID-19 were sometimes referred to as patients. This term has now been updated in our communications.

CBS: Decrease in mortality among people aged 80 years and up
Based on the national provisional counts of the number of deaths per week, Statistics Netherlands (CBS) estimates that more than 3,200 people died in week 8 (22 to 28 February). This is fewer deaths than expected for this period, and nearly 300 fewer deaths than in the week before (3,491). A significant decrease was seen in mortality among people aged 80 years and up and people receiving care under the Long-Term Care Act (WLZ).
RIVM also keeps track of mortality figures on a weekly basis, and has been tracking these figures since 2009 in cooperation with Statistics Netherlands. RIVM reports that mortality was elevated in the week of 18 to 24 February in all combined regions, with the exception of the Zeeland/Noord Brabant/Limburg region. A total of 3,348 deaths were reported in that week (354 more than the average expected mortality). We would normally expect to see between 2,830 and 3,157 deaths at this time of year. From 18 to 24 February, the reported mortality due to laboratory-confirmed COVID-19 was 322 deaths.
The method used by RIVM to estimate mortality is slightly different than the method used by Statistics Netherlands. Statistics Netherlands estimates excess mortality based on the average mortality per week in recent years, including increased mortality during unusual circumstances, such as a wave of seasonal influenza, or a heat wave. RIVM does not include the peaks during unusual circumstances in its calculations of average weekly mortality. The excess mortality estimates provided by Statistics Netherlands and RIVM complement each other.

That term is used for people who are so ill that they need to be hospitalised, or for people who became so ill that they died. Not everyone who tests positive for COVID-19 becomes so ill that they have to be hospitalised. Many people only develop mild symptoms, or do not notice any symptoms at all.
In the past, we have referred to people who tested positive for COVID-19 as patients. This term has now been updated in our communications.
